– All Competitors details including D.O.B. and emergency contact name and phone number
– School Representative Details, including emergency contact name and phone number
– A credit card for the payment of the entry fee (âŹ250/team) and the Pre-authorisation of the damage deposit (âŹ500/team)
It is the responsibility for the School Representative to ensure that this form is completed by the Parent/Guardian of all competing team members and lodged with the Race Office at Registration, prior to the start of racing. No team may commence racing until this form is submitted.
